Medical Records

Your Medical Records may be requested from the Health Information Management Department, located in Building F adjacent to the northwest corner of the hospital building. Hours of operation are Monday through Friday 8 a.m. to 4:30 p. m., excluding major federal holidays. You can reach us at (928) 348-3813. When visiting the Health Information Management department, please bring a photo ID with you. If the records you need are not available electronically, most requests can be filled within 7 to 10 business days. If it has been over 7 days please give us a call. However, factors such as record availability, size of the record and the volume of requests will affect response time. There may be a charge for the cost of reproduction of medical records. To obtain the records, complete a Medical Records Release Form, which is also available in Medical Records or you can print the form located below.

Request to Amend a Medical Record

A patient has the right to request an amendment to his or her medical record.  A physician has the right to determine if the change will be made.

A top priority OF MGRMC is to protect the security and privacy of patient information.  All information contained within the patient medical record is considered confidential and is federally protected under the Health Insurance Portability and Accountability Act of 1996. Medical Records shall not be released without the written authorization of the patient, or his/her legal representative (POA), a subpoena or court order.

f you are a current Patient Portal user, depending on the date of your last visit, you may be able to view some of your records online.

To access your records electronically you can enroll at myhealth.mtgraham.org (if not already enrolled).  To enroll you will need your Medical Record (MR) number. If you have difficulty creating your account for the portal, or for further information, contact the Medical Records department at (928) 348-4090, or email at www.MRInfo.org.;

If you need access for other family members please fill out the specified proxy for that individual.

Request a Birth certificate

Birth Records and paternity papers are obtained with the County/State in which you were born. Copies of these documents may be obtained from the Graham County Vital Statistics Office located at 820 W. Main Street or www.azdhs.gov Phone (928) 428-4441. The hospital is not allowed to release copies of Birth Certificates.
